Who must complete the forms?

Each new member must complete the forms, even if in a couple.  There is one exception: if in a couple, the new member pledge may be completed together.

Who will receive a copy of my submission?

  • A copy will be emailed to the Membership Director.
  • A copy will be emailed to you from UUSLO.org Website (website@uuslo.org).
  • A copy of your pledge form will be emailed to the Finance Administrator.
  • The information will be securely stored on the UUSLO.org website and the website administrator will have access and can resend emails if needed.

Is this information public?

No. The New Member Biography section of the UUSLO Membership Information form will be used to introduce you to the congregation in the newsletter and on New Member Sunday.

Your name, address, phone number and email address will appear in the member directory on the website, which may only be accessed by members and staff.

All other information is only available to UUSLO staff, the website administrator, and pastoral care members of the congregation.

This is a lot of information! Do I have to fill it out all at once?

No. There is an option to Save and Continue forms 1 and 2. If you click Save and Continue at the bottom of the form, you will be emailed a link to open the form and edit it at a later date. The link will expire after 30 days.

Your membership is not complete until you have submitted all the forms.

Can I make edits after I submit a form?

No. If you need to make changes, email the Membership Director at membership.uuslo@gmail.com.
